Use Adobe Media Encoder (it is automatically installed when you install Adobe CS or Adobe Premiere).
Or within Adobe Premiere, after editing, when you do Export, you will see 'Export Settings' (when Adobe Media Encoder is actually called). There is a crop tool near the top-left corner.
